Laboratory tests

Heavy metals testing

When an exposure is plausible, a targeted metal measurement can help answer a specific clinical question. The choice of metal, sample and timing matters more than ordering the widest possible panel. We interpret results alongside work, environment, diet and symptoms.

Source panel
An example panel measures selected metals in unprovoked urine using mass spectrometry, with creatinine-related reporting. Other exposure questions may require a different specimen.

In detail

Why this matters in your care

A focused history can identify an exposure worth investigating and help avoid results that are difficult to interpret. Relevant details include occupation, hobbies, water sources, fish consumption and recent supplements. The result is useful when it can guide exposure reduction or appropriate medical follow-up.

Reading the measurement

The example panel lists metals individually, including lead, cadmium, mercury and arsenic among others. Each has its own interpretation. Urine concentrations reflect excretion under the collection conditions and do not directly measure the total amount stored throughout the body.

Creatinine correction helps account for urine concentration but does not remove every source of variation. For some findings, the chemical form or a confirmatory specimen matters. A reference flag is a prompt for interpretation, not a diagnosis of poisoning.

Collection and next steps

Follow the collection instructions for the ordered panel and disclose medicines or products that could affect the result. The example discussed here uses urine collected before chelation. A urine sample taken after a chelating agent cannot be interpreted against an ordinary unprovoked reference range.

An unexpected result may require confirmation, exposure-source review or toxicology consultation. It does not automatically justify chelation or a “detox” programme.
