Laboratory tests

Toxicology and environmental exposure screening

An exposure assessment begins with a clear question about your environment, work, diet or symptoms. Selected laboratory panels measure particular chemicals or their metabolites. We explain what the test can establish before using a result to guide further investigation.

Urine panels
Selected environmental chemicals or metabolites, metals and, in some panels, mycotoxins. Each group has its own method and limitations.
A separate antibody method
A separate type of panel measures blood IgG/IgE antibodies related to mycotoxins. This is a different measurement from the concentration of a toxin in urine.

The panels differ by location. A measured exposure does not by itself show that a substance caused symptoms. Urine mycotoxin testing has important clinical-validity limitations; the sample and question must be specified.

In detail

Why this matters in HLC care

A credible exposure history helps select an appropriate test and plan a useful response. The aim is to identify a question that can change care, such as whether further occupational or environmental assessment is needed. A broad panel alone cannot explain every nonspecific symptom.

Different methods answer different questions

An example urine report separates mycotoxins, metals and environmental chemicals. Concentrations may be corrected for urine creatinine and compared with a reference population. A percentile flag shows where a result sits in that comparison; it is not automatically a threshold for toxic harm.

An antibody result describes an immune measurement, not the quantity of toxin in the body. It cannot be substituted for a chemical concentration or used to identify a building as the source of exposure. The actual method must be clear before interpretation.

Important limits of mycotoxin results

Urinary mycotoxins may be detected after ordinary dietary exposure, including in people without illness. These results do not, by themselves, diagnose illness caused by indoor mould. Commercial antibody panels also require careful appraisal of their clinical validity for the proposed question.

What a sensible follow-up looks like

Bring an exposure timeline, relevant work or environmental information, medicines and supplements. Follow the exact collection instructions. If a result is unexpected, the clinician considers confirmation, an alternative specimen or a specialist assessment before recommending any treatment.

A laboratory flag alone is not a reason for chelation, antifungal treatment or an intensive “detox” regimen. The next step should address the actual exposure and clinical findings.
